FBI Raid on Somali-Owned Home Health Agency Amid Minnesota Medicaid Fraud Probe
December 18 2025 BLOOMINGTON, Minn. — FBI agents raided a Bloomington business on Thursday, days after the State of Minnesota suspended the company’s license citing fraud concerns.
FBI agents and other federal investigators carrying boxes out of a business located in a plaza off 17th Avenue South near Old Shakopee Road East. The business operates in a suite next to a pizza shop, an Asian market, and a laundromat.
The business, identified as Ultimate Home Health Services, provides home and community-based services, the Minnesota Department of Human Services suspended the company’s license earlier this month.
A letter dated December 5 from the Department of Human Services states that the suspension was issued immediately after determining that individuals served by the program were at “imminent risk of harm,” and that the license holder and controlling individual were subjects of a pending administrative action related to fraud. The letter further states that state officials determined some clients did not require the services they were receiving, that a client death had not been reported, and that staff had provided false information to DHS licensors.
